  3. Most recent DE 9c Quarterly Wage Report (and payroll reports)
  4. Complete the Proprietor/Partner/Corporate Officer Eligibility Statement (for persons not listed on the DE 9c or payroll reports)
  5. Complete Employee Enrollment forms
  6. Provide documentation for each employee who declines coverage for any family member
  7. 1st month premium payment (check latest rates here)
